RESUMEN

Taenia solium taeniasis/cysticercosis (TSTC) is a parasitic zoonotic disease that is endemic in several developing countries, causing serious public health and economic impacts. A cross-sectional study was conducted to assess knowledge, attitudes and practices (KAP) related to porcine cysticercosis (PCC) transmission, prevention and control among smallholder pig farmers in Kongwa and Songwe Districts in Tanzania. A semi-structured questionnaire was administered to 692 smallholder pig farmers from randomly selected households. STATA software version 17 was used to analyse quantitative data, summarize farmers' KAP about PCC and calculate performance scores. Nearly half (42%) of the respondents had little knowledge regarding PCC, only 17% of the respondents had good practices towards prevention/control of PCC and 72% had a positive attitude towards PCC- prevention/control measures. The majority (73%) of smallholder pig farmers admitted deworming their pigs regularly, whereas 76% reported deworming themselves and their family members regularly. Albendazole and ivermectin are the most commonly used medications for deworming people and pigs, respectively. According to the findings, the majority of smallholder pig farmers in Kongwa and Songwe Districts showed a good attitude towards PCC prevention/control measures but had limited knowledge of the PCC life cycle and control. In addition, only one in five farmers was engaged in good practices. The findings revealed further that farmers are engaged in risky behaviours that aid the spread and perpetuation of the T. solium parasite in the study area. It is recommended that farmers should be given proper health education on the T. solium transmission cycle and preventive/control practices to limit PCC transmission.

BACKGROUND: Brucellosis is a bacterial zoonosis of public health and economic importance worldwide. It affects a number of domestic animals, wild animals and humans. Human brucellosis originates from either livestock or wildlife. The species of Brucella circulating in wild animals in Tanzania is largely unknown due to insufficient surveillance. This study was carried out to identify Brucella species found in selected wildlife hosts in the Serengeti ecosystem. METHODOLOGY: The study used a total of 189 archived samples that were obtained from cross-sectional studies previously conducted between 2000 and 2017 in the Serengeti ecosystem in Tanzania. Whole blood, serum and amniotic fluid collected from buffalos, lions, wildebeest, impala, zebra and hyena were available for DNA extraction. Multiplex polymerase chain reaction for B. abortus, B. melitensis, B. ovis and B. suis (AMOS PCR) and quantitative real-time PCR (qPCR) targeting the bcsp31 and IS711 genes for Brucella genus detection and the IS711 targets alkB for B. abortus and BMEI1162 for B. melitensis were used to detect Brucella strains. RESULTS: Out of the 189 samples tested, 12 (6.35 %) and 22 (11.6 %) were positive to AMOS-PCR and qPCR, respectively. Most of the positive samples were from lions (52.6 %) and buffaloes (19.6 %). Other animals that were positive included: wildebeest (13.6 %), impala (13.6 %), zebra (4.5 %) and hyena (4.5 %). Out of 22 positive samples, 16 (66.7 %) were identified as B. abortus and the other six samples did not amplify for neither B. abortus nor B. melitensis. CONCLUSIONS: The detection of Brucella DNA in archived wild animal samples shows testing potential of samples collected from this population. The zoonotic species B. abortus and B. melitensis detected in wild animals have previously been reported in livestock and humans in the region. The findings suggest that, due to the contact network, some of the identified wild animal hosts in this study could be reservoirs for infections in domestic animals and humans within the Serengeti ecosystem while others are likely dead-end hosts. One Health control strategies and continuous surveillance programs in other wildlife reserved areas should be implemented to help predicting transmission in livestock and humans in the region.

Human exposure to persistent organic pollutants (POPs) begins during pregnancy and may cause adverse health effects in the fetus or later in life. The present study aimed to assess prenatal POPs exposure to Tanzanian infants and evaluate the distribution of POPs between breast milk, maternal blood, placenta and cord blood. For assessment of prenatal exposure, 48 maternal blood samples from Mount Meru Regional Referral Hospital (MMRRH), Arusha Tanzania, were analyzed for organochlorine pesticides (OCPs), polychlorinated biphenyls (PCBs), brominated flame retardants (BFRs), dioxin-like (DL) activity and perfluorinated alkyl substances (PFASs). For evaluation of POPs distribution between maternal/infant compartments, breast milk, placenta and cord blood corresponding to the maternal blood were analyzed for OCPs, PCBs and BFRs. In maternal blood, p,p´- DDE was detected in 100% of the samples ranging between 29 and 1890 ng/g lipid weight (lw). Milk quality depends on the physicochemical characteristics, hygienic standards, and nutritional quality; however, animal husbandry practices, unhygienic harvesting and processing, may affect its quality. A cross-sectional study was conducted between August 2010 and July 2011 to assess the hygiene of cow milk production environment, raw cow milk physicochemical characteristics, and microbial quality and estimate the prevalence of antimicrobial residues using standard methods in Pemba Island. A total of 98 raw cow milk samples from selected smallholder dairy farms were analyzed, and the judgement on the quality used the East African Standards. Generally, the milk production chain was done under the unhygienic condition, and dirty plastic containers were used for collection and storage of milk under room temperature. Some milk samples had abnormal colour (2.1%), abnormal smell (7.1%), and pH below normal (35.7%), clotted on alcohol test (9.2%), and had the specific gravity below normal (13.3%). All the milk samples had mineral contents within the recommended range. Milk samples with butterfat below normal were 29.6%, while 14.3% had total solids below recommended values. The mean total viable count (TVC) of milk container surfaces was 9.7 ± 10.5 log CFU/100 cm2, while total coliform count (TCC) was 7.8 ± 8.5 log CFU/100 cm2. Up to 55.1% of milk had TVC beyond the recommended levels. The milk mean TVC was 11.02 ± 11.6 log CFU/ml and TCC was 6.7 ± 7.3 log CFU/ml. Up to 26.5% of milk samples had the TCC beyond levels. Results on physicochemical characteristics and nutritional analysis show that the raw cow milk in Pemba Island is of inferior quality. Microbiological results of this study imply heavy contaminations of milk. Antimicrobial residues were detected in 83% of the samples and most of them were from Wete District. Unhygienic milk production chain accelerates microbial contaminations, and antimicrobial residues in milk are a big problem that needs urgent attention from the responsible authority.

A cross-sectional study was conducted to determine the occurrence of indigestible foreign bodies (IFB) in cattle slaughtered at Morogoro Municipal Slaughterhouse, Tanzania. A total of 387 slaughter cattle were examined for presence of IFB. Out of 387 examined cattle, 93 (24.03%) had IFB in their forestomachs. The observed IFB were plastic bags, fruit seeds, clothing materials, ropes, hairballs, leather materials, stones, metallic nails, and wire. Plastic bags were the most frequently (50.5%) observed IFB followed by fruit seeds (18.3%). A significantly (p < 0.05) high proportion of old animals (31.7%) had IFB compared to the young animals (21.2%). Similarly, the frequency of occurrence of IFB was significantly high (p < 0.05) in crossbred dairy cattle (42.3%) compared to local breeds (22.7%). Cattle that appeared with poor body condition (37.8%) were found to be more affected (p < 0.05) by IFB than those with good body condition (15.9%). In 91.4% of animals which had IFB, all the materials were located in the rumen. This study showed that presence of IFB is a common problem in cattle slaughtered at Morogoro Municipal Slaughterhouse and may significantly cause poor production and mortality in affected animals. Therefore, appropriate solid waste disposal should be implemented.

This is the first study to report organochlorines (OCs), including chlorinated pesticides (OCPs) and polychlorinated biphenyls (PCBs) in human milk from Tanzania. The main aims of this study were to assess the level of contamination and the possible health risks related to OC exposure in nursing infants from the Northern parts of Tanzania. The environment in the northern part of Tanzania is influenced by rapid population growth, and increased urbanization. Urban agriculture is common and of economic value for low income families. In Arusha, many households sell eggs from free-ranging backyard chicken. In 2011, 159 eggs from different households in five different locations in Arusha were collected, homogenized, pooled into 28 composite samples and analyzed for a wide selection of POPs. Levels of POPs varied widely within and between the locations. The levels of dieldrin and ΣDDT ranged between 2 and 98,791 and 2 and 324ng/g lipid weight (lw), respectively. EU MRLs of 0.02mg/kg dieldrin for eggs were exceeded in 4/28 samples. PCBs, HCHs, chlordanes, toxaphenes and endosulfanes were found at lower frequency and levels. Brominated flame retardants (BFRs), e.g polybrominated diphenylethers (PBDEs), hexabromocyclododecane (HBCD) and 1,2-bis(2,4,6-tribromphenoxy)ethane (BTBPE) were present in 100%, 60% and 46% of the composite samples, respectively. In Tanzania fish is one of the most important protein sources for the rapidly increasing population. Wild fish is threatened by overfishing and pollution from agriculture, industries, mining, household effluents and vector control. To monitor possible implications for public health, the geographical differences of the occurrence and levels of persistent organic pollutants (POPs) in tilapia fish (Oreochromis sp.) from four different Tanzanian lakes were investigated in 2011. Concentrations of organochlorine pesticides (OCPs), polychlorinated biphenyls (PCBs), polybrominated diphenyls (PBDEs) and hexabromocyclododecane (HBCDD) were determined in pooled samples of tilapia muscle from Lake (L) Victoria, L. Tanganyika, L. Nyasa (also called L. Malawi) and L. Babati in Tanzania in 2011. Levels of Σ-DDTs (274 ng/g lipid weight (lw)) and sum of 7 indicator PCBs (Σ-7PCBs) (17 ng/g lw) were significantly higher in tilapia from L. Tanganyika compared to the other lakes. The highest levels of Σ-endosulfan (94 ng/g lw) were detected in tilapia from L. Victoria. Toxaphenes were detected in low levels in fish from L. Tanganyika and L. Babati. Results revealed a geographic difference in the use of DDT and endosulfan between L. Victoria and L. Tanganyika. Low ratios of DDE/DDT in tilapia from L. Tanganyika indicated an on-going use of DDT in the area. Median levels of ΣBDEs, including BDE-209, were highest in L. Victoria (19.4 ng/g lw) and BDE-209 was present in 68% of the samples from this lake. The presence of BDE-209 indicates increasing influence of imported products from heavy industrialized countries. The measured POP levels in the studied tilapia were all below MRLs of EU or were lower than recommended levels, and thus the fish is considered as safe for human consumption. They may, however, pose a risk to the fish species and threaten biodiversity.

An abattoir survey was conducted on clinically healthy 115,186 cattle, 61,551 sheep and 37,850 goats slaughtered in Arusha municipality, Tanzania, between 2005 and 2007. The objective was to determine the prevalence of disease conditions affecting the lungs. Routine meat inspection procedures were used to detect the presence of the pathological lesions. A total of 15,245 (13.2%), 4668 (7.8%) and 3192 (8.4%) lungs of cattle, sheep and goat, respectively, were condemned due to nine diseases/conditions namely pneumonia, hydatidosis, emphysema, abscesses, anthracosis, pleurisy, calcified cysts, melanosis and bovine pulmonary tuberculosis. Pneumonia was the leading cause of condemnations as it was responsible for 4594 (30.1%), 1495 (31.4%) and 1072 (33.6%) of all the condemned lungs in cattle, sheep and goats, respectively. Anthracosis, pleurisy, melanosis and pulmonary tuberculosis were only recorded in cattle attributing to 7.3%, 6.4%, 2.9%, and 0.7% of lung condemnations, respectively. The percentages of lung conditions recorded in 2005, 2006 and 2007 were not statistically different (P>0.05) among cattle, sheep and goats. The different lung lesion percentages recorded in cattle were significantly higher (P<0.05) compared to sheep and goats. There were no significant (P>0.05) variations between lung condemnation percentages due to various diseases during the rainy and dry seasons. Because of their zoonotic nature, occurrence of hydatidosis and tuberculosis may pose a public health risk. The study showed that different lung diseases/lesions in domestic ruminants are prevalent in Tanzania. Thus, there is a need to introduce appropriate control measures of diseases affecting lungs to minimize the rate of infection and reduce the ensuing economic losses. The data obtained from this survey cannot be wholly relied upon as accurate, but it can be used as a baseline for more extensive epidemiological investigations.

Occurrence of antimicrobial residues in commercial chicken eggs was determined in Morogoro municipality between January and February 2007. Twenty smallholder farmers were interviewed on the types of antimicrobials, reasons of use and their awareness on antimicrobial withdrawal period. Seventy egg samples were collected for qualitative antimicrobial drug residues analysis by use of agar well diffusion and Delvotest SP assays. It was found that farmers use antimicrobial drugs as prophylaxis and treatment of common chicken diseases namely fowl typhoid (85%), infectious bursa disease (Gumboro) (65%) infectious coryza (65%), collibacilosis (55%), coccidiosis (54%), Newcastle disease (50%), helminthosis (20%) and fowl pox (15%). Antimicrobials accounted for 85% of the drugs commonly used. It was also found that 65% of the farmers treat their chicken themselves. The common drugs were oxytetracycline (75%), egg booster (50%), amprolium (35%), sulphamethoxypyridazine (35%), sulphanilamide (25%), chlortetracyclines (10%), chloramphenicol (10%), sulphadiazine-trimethoprim (20%), duoxycycline (20%), sulphadiazine (25%) and flumequine (10%). Eighty per cent of the farmers had knowledge on antimicrobial withdrawal period sold eggs before withdrawal period and almost 85% were unaware of possible effects of antimicrobial residues in humans. All 70 eggs were positive to antimicrobial residues by Delvotest kit, but 21.4% positive with agar well diffusion test. It was concluded that the presence of antimicrobial residues in table eggs could be of public health significance to the egg consumers in Morogoro municipality.

A 13-year (1990-2003) retrospective study was carried out to investigate the occurrence of cystic echinococcosis (CE) in patients at Wasso hospital in Ngorongoro district, Tanzania. Hospitalization records were reviewed and confirmed cases of CE were classified according to ward of residence, age, sex, cyst location and type. A total of 171 hydatidosis patients were diagnosed and operated giving an average of 13 cases per year, equivalent to approximately 10 cases per 100,000 people per year. Cystic echinococcosis affected more females (59.1%) than males (40.9%). A significantly higher number of CE cases were recorded in individuals <30 years of age (22.8-30.4%) than in those >30 years of age (2.3-9.4%) (P < 0.05). High proportion of CE (81.5%) occurred in the liver than in other sites, and more females had higher hepatic cysts (47.4%) than males (31.0%). Solitary cysts and single organ involvement were noted in 81.5% cases and multiple cysts in 18.5% cases. Higher multiple organ infections (14.1%) and cyst recurrences (5.3%) were also recorded in females. Orgosorok ward had the highest number (18.7%) of CE cases, while Nainokanoka had the lowest (1.2%). It was concluded that CE infection occurs frequently in humans in Ngorongoro district and female and youths are those most affected. These findings merit for more extensive epidemiological investigations of CE in human to better determine the prevalence, economic impact and risk factors for the disease in this district and other areas of Tanzania.

A 3-year (2005-2007) retrospective study was carried out to investigate the occurrence of cystic echinococcosis (CE) in cattle and shoats (sheep and goats) slaughtered at Arusha municipal abattoir, Tanzania. A total of 115,186 cattle and 99,401 shoats were slaughtered. Cattle liver, lungs, spleen and heart condemnation rate was 16.35%, 13.04%, 2.09% and 3.06% respectively while 17.63%, 7.63%, 0.38% and 0.04% of shoats' liver, lungs, spleen and heart respectively were condemned. A highly significant (p < 0.001) cystic echinococcosis (CE) infection rate was recorded in shoats (6.02%) than in cattle (4.2%) probably because of differences in grazing patterns. Cattle lungs were more affected by CE (22.5%) than liver (19.7%) while shoats liver were found to be more affected (21%) than the lungs (19.3). A significant (p < 0.001) higher CE infection rate was observed in cattle spleen (15.5%) than shoats (9.7%). There was no significant (p > 0.05) difference between condemnation rates due to hydatidosis during rain and dry seasons probably because of chronicity of the disease. The study have established that CE is prevalent in animals slaughtered at Arusha abattoir and causes higher condemnation rates of edible offals. This merit for more extensive epidemiological investigations to better determine the prevalence, economic impact and public health importance of the disease in the region.

A study on the prevalence of hydatidosis in cattle, goats and sheep was carried out in Ngorongoro district of Arusha region, Tanzania. A 4-years data records from four slaughter slabs were retrieved and analysed. In addition, meat inspection was done in the same slaughter slabs for nine months and 64 households were interviewed to assess the community awareness on hydatidosis. Results showed the overall prevalence of hydatidosis to be 47.9%. Species prevalence of 48.7%, 34.7% and 63.8% in cattle, goats and sheep respectively was recorded. Of 174 cysts examined in cattle, 37 (21.3%) were fertile, 126 (72.4%) were sterile and 11 (6.3%) were calcified. Out of 215 goats and 67 sheep cysts examined, 52 (24.7%) and 26 (38.8%) were fertile, 138 (64.2%) and 38 (56.7%) were sterile, 24 (11.2%) and 3 (4.5%) were calcified respectively. The higher percentage of fertile hydatid cysts in sheep and goats coupled with the practice of backyard slaughter of sheep and goats suggests that, these animals could be important intermediate hosts for the maintenance of the domestic life cycle of E. granulosus in the locality. Questionnaire survey revealed that 17.2% of the respondents were aware of hydatidosis but non of them were knowledgeable on its transmission. Up to 84.4% of the respondents had domestic ruminants and donkeys, while 89.1% had dogs. Of the households with dogs, only 19.3% had their dogs dewormed at least once in life time. Most of the households (87.7%) had their dogs managed freely and 77.2% of the respondents reported school children to be the closest friends of dogs in the family. The prevalence of E. granulosus infection in wildlife and the possible relationship of the domestic cycle to the sylvatic cycle operating in the same area are unknown and need to be studied.

Prevalence of thermophilic Campylobacter infections in humans, chickens and crows was determined in a cross-sectional study that was carried out in urban and rural areas of Morogoro region, Tanzania during the period of January 2003 to December 2004. A total of 632 human stool samples, 536 cloacal swabs from local and broiler chickens and 22 intestinal contents from crows were screened for presence of thermophilic campylobacters using Skirrow's protocol. Representative Campylobacter jejuni isolates from human and chicken samples were also analysed by polymerase chain reaction (PCR) as a definitive identification method. The overall prevalence of thermophilic campylobacters was 9.3% (95% CI: 7.2-11.9), 69.8% (95% CI: 65.7-73.6) and 72.7% (95% CI: 49.8-89.3) in humans, chickens and crows respectively. In humans, 59 thermophilic campylobacters were isolated of which 96.6% were C. jejuni and 3.4%Campylobacter coli. There was a significantly (P<0.001) higher prevalence in young individuals (16%) than in adults (7%). Of 341 isolates from chickens, 91.2% were C. jejuni and 8.8% were C. coli. A significantly (P<0.05) higher infection rate was observed in rural local chicken (76%) than in broilers (60%). In crows, of 16 isolates, 93.8% were C. jejuni and 6.2% were C. coli. Definitive identification of C. jejuni by PCR revealed positive results in 74.1% of 243 analysed isolates. Findings in this study indicate high prevalence of thermophilic campylobacters in humans, chickens and crows in Morogoro, and a higher infection rate of C. jejuni than that of C. coli in different animal species. Age of humans and location of chickens were identified as risk factors for thermophilic Campylobacter infections. Positive isolates to biochemical tests that indicated negative results on PCR indicates the additional value of PCR for definitive diagnosis of C. jejuni.
